Black Communities and Healthcare | Individualize Care Welcome Back & Happy BLACK HISTORY MONTH! Continuing our theme for the month of February, I would like to highlight Black Communities and Healthcare and today let’s discuss how we can start to individualize care in vulnerable populations.
I would like to introduce a technique I created that can be applied to ANY population or demographic. Let me briefly outline the technique, The A-Cycle of Patient Engagement:
ASSESS: As clinicians, we are always assessing our patients but in this step let’s assess ourselves as the providers. Let’s take an objective look at our delivery of Care, evaluating our facility & staff.
ACTIVATE: This step makes room for open conversations with our patients. This is a time to hear their pain points, collaboratively goal set, & empower in their healthcare journey
ADVOCATE: In this step, we connect the dots for our unique population. There’s a number of ways to advocate. With your demographic in mind, what are ways you could advocate for them?
ADJUST: Lastly, with any process there are adjustments & pivots. That’s why this is a cycle & not linear. Linear implies there’s a start & end but with patient engagement & activation, we have to remain agile in our efforts. People change & therefore our approach has to well.
